Supported Housing in Stockton

Our Stockton staff have lots of experience working with people in many settings (e.g. youth work,
mental health, hostels, domestic violence, drug and alcohol services, welfare rights) and are all passionate about empowering people to realise their potential.

Properties

  • 30 supported tenancies.
  • All properties fully furnished.
  • 24 single and 6 shared tenancies.
  • Located in central Stockton.
  • We do not provide a 24 hour support/call out service.
  • 7 tenancies are non commissioned and 2 are general needs tenancies.

Referral Criteria

  • Aged 16 plus.
  • Homeless or in housing need.
  • The person must be willing to engage, participate and work with the service.
  • Support needs appropriate for the service.
  • Has means of paying rent or accessing housing benefit.
  • Does not require personal care from CC87.
  • Does not pose a risk or threat to other service users, members of staff.

Referral Process

  • For commissioned properties referrals from Housing Options, Stockton Borough Council or the Leaving Care Team.
  • Application form needs to be completed by applicant or referral agency.
  • Applicant will be invited for an interview/needs assessments.
  • Risk assessments will be sought from referral agency.
  • Decision will be made upon application once sufficient information is gathered.
  • Applicant and referral agent will be advised in writing of the decision.
  • All applicants have the right to appeal decisions.

Location Criteria

  • Suitability to property.
  • Level of housing need.
  • Compatibility with fellow tenant (for shared properties).
  • Level of support needs.
  • Length of time on waiting list.
  • Current circumstances.

The Support Service

  • Office opens Monday to Friday 9am to 5pm.
  • Open door policy.
  • Each person has their own designated Tenant Support Worker.
  • Support with managing a tenancy, maximising income, budgeting skills, meaningful use of time, physical andmental health, motivation and taking responsibility, alcohol and substance misuse, self care and living skills.
  • Planned/structured support in the person’s home.

Support Plan

  • Tenant Support Worker and client complete an individual support plan/programme within the first 2-4 weeks.
  • Tenancy based support to develop the skills and confidence to manage a tenancy independently and successfully.
  • Support plan constantly reviewed to reflect needs and circumstances of client.
  • To help assess when the client is ready to move on into their own tenancy with other housing providers.

Move On

  • Assessed as being able to manage a tenancy independently.
  • Support and guidance to source move on accommodation.
  • Can provide resettlement support for first 4 weeks of new tenancy or refer for additional support if needed.
  • Open door policy thereafter.
