Supported Housing in Hartlepool

Our Hartlepool staff have lots of experience working with people in many settings (e.g. youth work,
mental health, hostels, domestic violence, drug and alcohol services, welfare rights) and are all passionate about empowering people to realise their potential.

Properties

  • 12 Supported Properties and 9 RSAP properties as well as 8 General Needs accommodations.
  • All supported properties are full furnished, with white goods/ sofa/TV stand/bedroom furniture.
  • All Supported RSAP properties are fully furnished minus a TV.
  • Support is intensive and client-focused.
  • Single and shared tenancies.
  • Service is provided during office hours.
  • Decorated and maintained to a high standard.

Referral Criteria

  • Referrals accepted for age 16 years plus.
  • Homeless or are in housing need.
  • The person must be willing to engage with the support service offered.
  • That the persons support needs can be met by the support service offered.
  • Has access to housing benefits or other means of paying rent.
  • No personal care provided by CC87.
  • No risk to staff or other tenants.

Referral Process

  • Applications are accepted through agencies or self-referral.
  • Applicants are invited for interview to assess for suitability for the service.
  • Risk assessments will be sought from agencies working with applicant.
  • Decisions will be made once all information gathered. Applicant and referrer will be informed of decision.
  • All applicants have a right to appeal decision.

Location Criteria

The Support Service and Support Plan

  • Office opens Monday-Friday 8.30am-5:00pm.
  • Each tenant has own designated support worker.
  • Each tenant works with support worker to create individualised support plan and goals which is reviewed 12 weekly.
  • Individualised support is centred around successfully managing tenancy/ promoting independent living skills. Maximising income/budgeting, sourcing activities/hobbies to promote skills and learning, physical/ mental health support, accessing specialist services, addictions.
  • Support is carried out in the tenants home environment as well as office or external places.
  • Access and discuss with tenant when ready to move on.

Move On

  • Assessed as ready to manage tenancy independently
  • Access move on accommodation through council or other providers, including private accommodation
  • Provide structured continued support during move on transition for up to 6 weeks, with an open door policy afterwards
